ONLINE Bitch Nomination for EESSC Futurity
I hereby nominate the following dam, thus making her puppies eligible for enrollment in the EESSC Futurity. Nomination fee is $11.00 for dams nominated before puppies are whelped. (Nominations are accepted after the puppies are whelped up to 60 days of age for a late fee of $16.00) Only the owner or lessee of the bitch may make the nomination.

ONLINE Puppy Enrollment for EESSC Futurity
This form must be submitted by 7 (seven) months of age. The fee after that is $50.00 per puppy. (Must be received no later than May 1)
